Caregiver's Affidavit & Information

 

A Caregiver's Authorization Affidavit is a form you fill out to enroll the child in school or get medical care for the child. California law states who can use the form and what information should be on the form.  The Caregiver must be a resident within CUHSD boundary lines and the student must reside with the Caregiver 7 days a week, 24 hours a day. Caregiver status must be renewed every school year. 

NOTE: Caregiver status does not necessarily mean Educational Rights. Site and District Administration must review all submissions.
